I would like to extrude tubing from this material. Does anyone know where information is regarding material suppliers, processing guidelines, grade options ???

Go to http://www.matweb.com/.  You'll have to register, but it's free.  Once you are in search for material types.  Under polyethylene you'll find low density (LDPE), linear low density, medium density, and high density PE.  the Website will not only give general properties, it includes manufacturers.  You can also find information on specific grades of materials.  I use it frequently.

X-linked PE as such cannot be extruded.
Intermediated products which are crosslinked afterwards can be bought commercially. Techniques available are Sioplas compounds (from for example AEI), Monosil extrusion techniques (Nextrom, Maillefer) and vinylsilane-PE copolymers (Borealis).

Highly crosslinked materials can be manufactured by continuous vulcanisation tubes (steam, nitrogen, salt) under high pressures and temperatures (200 - 300°C)
