Who is responsible for a fireplace/chimney design.  There are elements in the design that fall outside of my area of expertise.  Are the "guts" (flue liner, damper, etc.) typically designed and detailed by a delegated engineer while I provide the masonry/reinforcing/seismic design requirements?

The guts as you say are spec'd by Architectural standards per se, with any special reinforcing for the masonry block provided by you.  Sometimes even that falls under a standard.  I have found the foundation to be the area I mostly get involved in, plus tying the whole chimney structure into the rest of the structure.
