I am looking for a UL listed - FM approved butterfly valve with electronic actuator.

The application is to fill a storage tank upstream of a fire pump and tie the electronic actuator to level controls in the tank.  The tank is sized for the required volume (30 minutes of 1000 gpm pump operation = 30,000 gallons).

I want to be able to fill the tank automatically whenever the water level goes down - after pump and/or sprinkler tests.

Is there such a combination of valve and actuator that is UL/FM?

You're asking for a butterfly valve with a motor operated valve actuator. I have never seen a motor operated valve actuator listed for fire protection service.

My initial concern with any MOV is what state does the valve fail in if power is lost - for this and other reasons I don't think such an animal exists for fire protection service (at least I've never seen one).

UL / FM listed in what fashion? This is for your water supply and not the fire protection system per se correct? The tank, like a municipal supply does not require equipment listed for fire protection service (as per NFPA) to maintain water levels within the tank, though ultimately that call will be made by your AHJ. There are some pretty fancy, heavy duty, motor controlled industrial valves out there.

The valves are upstream of the tank but downstream of the DCDA so we have been told this is not the water departmet's jurisdiction.

The AHJ is citing NFPA 22 (2003) 13.4, which when I read it doesn't seem to really apply to our case.

The valves would fail close so that the tank would not overfill which is the only purpose of the valves.

I haven't been able to find a combination but wanted to put it out to those with more experience.
