Figure 6-9 (page 52) shown 4 wind load case, but it seems most design only used case 1 in load combination. When we need to use all 4 case?

All 4 cases are required all the time.

But you can use your judgement as to which actually has the most effect on the element you are designing.

Yes, you have to use them all, all the time.  This is part of the reason for the absurd number of load combinations that are generated.  The last time I did a combination generation in RAM, it was over 300.  300!!!

300? Were you using Wind and Seismic and Snow and Flood?

I typically end up with around 70-80 with Wind loads.

No flood, just wind, seismic, dead, live, and snow.

If you are doing direct analysis on a building that requires the notional loads to be used in combination with the lateral loads (i.e. has a stability amplifier > 1.5) you can end up with a lot more - playing around with Ram's combo generator results in...get this...1252 combos!!!!
