I am developing a procedure for overlaying Inconel 625 weld on an Incoloy 825 base. I am using QW-453 and QW462.5(d) as the guide for what I need to do. Base plate is 16mm, and I am overlaying with 3 layers of 4.0mm SMAW.

For the procedure testing, QW-462.5(d) is a little unclear. Do I have to carry out 2 side bends and 2 longitudinal bends (side bends are fine but the longitudinal bends are giving me a problem)? As an alternative, may I carry out 4 side bends as suggested by QW-453 note (5)?

Yes.  The keyword in note 5 is "either".

Most opt for this instead of longitudinal bends.
