Can anyone point me to a document that gives guidelines for derating cable for altitude?

I don't think such standard exist [since NEC, VDE 0298, IEC60287, UL83 do not consider such a derating]. But, of course, from 1 km above the sea level a derating will be necessary -for the cable in open-air or exposed conduit mainly. There are manufacturers that recommend 2% reduction per 100 m above 2 km and the reducing factor may reach 30% for 20 km above the sea level.

The article indicated by winterpeg1 is very good but is limited to 4000 m. [I think also that the sign ">"=more has to be "<" =less than].
Nevertheless may serve as a good indication.
