The manufacturing method is the exact same as machine screws: cut a blank from wire, forge the head/shank features, roll threads.

Unlike "normal" machine screws, the "pre-threadroll" (as cold headed) shank diameter is not round, but has a cross section that will generate the desired "trilobular" thread shape/dimensions after thread roll. These screws are also typically case hardened.
