5,380 sf developer retail shell.  the shell upfit drawings show two single user bathrooms (1 lav, 1 gravity flush toilet) plus 1 mop sink & 1 tank hot water heater.  

is a backflow preventer required by code right after the water meter?

Look into the building codes for the requirements.  Individual fixtures such as toilets likely require backflow preventers or air gaps.  Design the potable water supply system in a manner that prevents contamination from non-potable liquids, solids or gases being introduced into the potable water supply through cross-connections or any other piping connections to the system.  Install backflow preventers in accessible locations suitable for inspection and testing.  I don't know about the requirements at the supply but it sounds reasonable.
