What would be an appropriate fee for writing the specs and confirming the correct installation for a 450 - 500kw back-up generator (expected cost of around $ 180K) for a small county-owned hospital?  Can consulting fees be charged instead of a percentage of the project?

You can quote it anyway you want to.

Estimate your hours add a buffer.

Try to foresee any gotchas.

Figure your rate.

The gotchas are many on a hospital project.  Check the codes, testing and commissioning requirements for the application.

Be very specific in your scope of work.
