Heat Exchanger Tubes Leak

(OP)
Hi All

I have one reboiler heatexchanger working in plant for 15months. The problem start appear on the month of 14. The tubeside medium is reflux hydrocarbon (ethane) about 35barg. On the shell ia R22 refrigeration medium working at about 17bargs. This reboiler start block/cloged after opening and checking found face of tubesheet and tube weldment leak. And the more scare is the thickness gaging shows thicknes 3.1 3.0 1.2 1.5 randomly for 15 tubes. The tube/tubesheet are  carbon steel. Its can be concluded that the corrosive happen very rigously - its suppose not !( I believe).Straight tube double bonnet type of hx.

From above, can expert advise me what was suspected happen in this HX?.

Thanaks
