I have a direct drive pulley system that has a relatively large cable size wrt the pulley diameters.  Should I take the cable thickness into account in my calculations?

Calculations should always use the pitch line of the cable, wherever it is.

E.g. for a cable over a 'flat' pulley or right circular cylinder, the effective radius is the pulley radius plus the cable radius.

In addition to above, make sure that you do not bend the roping system below the minimum bending radius.
