Does the load duration factor (Cd) of the NDS apply to wood connections using nails, screws, and/or bolts?  My copy is a bit outdated and the page with that should have that info on it is blank.

Yes, all connectors are given base design values (Z and W) and you have to apply all the applicable load adjustment factors (not only Cd but all the rest that are indicated in the connection table (Table 7.3.1 in the NDS 97 and Table 10.3.1 in the NDS 2005).
