The longer answer is that these symbols aren't often used together, as in most cases either the position or the concentricity defines a boundary that covers both cases. This happens when you specify a modifier, like maximum material condition, for the position tolerance. The modifier isn't normally used for concentricity.
That said, I'm not claiming to be an expert in applying GD&T, but I've seen a lot of incorrect symbols on prints from our customers, causing confusion in interpretation.
I hope this helps, but double check my advise - or someone here can confirm or correct my comments.

I don't think that is a standard. If I were to make that part, I would know which way to go. I suggest go by the regular standard instead of reinventing it.

If concentricity was added, the machine shop and inspection would probably call and question what exactly is wanted. The cost of the part would probably go up also.
