How do you add/remove buttons from that little toolbar?

Also, is there a key I can press and hold that would stop the menu from popping up while I'm selecting things in the history tree?

You can't and there isn't.  The best you can do is to go to Tools/Customize and change the two options that they give you for Context Toolbars there.  It sure would be nice to be able to customize those little buggers.  Please submit an enhancement request, and maybe someday SW will listen.
