One of my users is getting a warning when he tries to insert a screw into his assembly. He gets the following warning message:

A document named "Flat Head Screw_AI" is already opened. Do you want to show this already-opened document?

He only has his one large assembly open, nothing else is open that we can tell.

Any suggestions?

Probably somewhere in the large assy, a document named "Flat Head Screw_AI" exists, or is referenced, in the assy or one of its components.

Do a search on the assy, or use the Find References option.

That file holds the different configurations of flat head screws in the toolbox.  Inserting a flat head screw when you already have one in your model is a common and supported activity.  The message box is unusual and suggests an incorrect setting in the toolbox.  To start with you may want to check that both:
  • insert Documents as read only
  • always change read only status of document before writing
are selected in the toolbox properties.
