Go to Tools -> Customize... and while you're in the 'Toolbars' tab, select 'New'.  You will then be asked to assign a name to the toolbar and to select with applications that you wish the toolbar to be available.  Once you assign a name and make any changes to the application list, hit OK and a small empty toolbar will appear on the screen.  Now go back to the Customize dialog and select the 'Commands' tab.  Now you can drag any command and/or icon from the various command list to create the toolbar content that you wish.  And if you're adding customized commands for NX Open routines, or Journals, or whatever, you go to the bottom of the list of 'Categories' and select either 'New Button' or one of the 3 styles of 'My Buttons' which you can control all aspects of.

For some Help with these procedures, while the Customize dialog is open, press the F1 key and the on-line help will come up.

OH, one other thing.  In order to save you changes, after you create and place the toolbar where you want it on your desktop, remember to save your Role.
