Problems with oil filled transformer (GSU) clamping spacers

Problems with oil filled transformer (GSU) clamping spacers

(OP)
I have to make a presentation about the problems with an oil filled transformer (GSU) on our site. One troubling issue is with the clamping spacers that were used with this particular model during the original construction - because of their low density they are subject to failure over time.

I'm looking for some pictures or some detailed drawings to help demonstrate the problem...any help out there?  a link to a picture, or sketch, an attachment?

Has anyone else ever heard of such a problem?
