Self Employment Fee Scales

(OP)
If you are self employed, what have you found to work well as a fee structure?  By the hour for modeling, price per drawings,CD's, & COSMOS extra fees?  What about if your just converting models to SW from another CAD program (no engineering involved).  What if a company comes to you and says I need a machine to do "this" and it must be within the specifications, etc?  Anyways I know there are many variables here but if you have found things that work or that don't work please reply with actual numbers, ranges, or recommendations, etc...

Also what about liability?  Do you have forms for a prospective client to fill out that relieves you of certain liabilities, etc...Sorry this was an afterthought.
