Does anyone know where that table of the volume of leaks at diff. pressures and hole sies. I thought it was in an EPA report.

i seem to remember seeing this in an AWWA publication many years ago. If you, or a friend, is a member try their website.  Also, many local water suppliers have this information and provide it free to their customers. We do when I worked for my City, Salem, Oregon.

You can also calculate it using an orifice equation.
