I have discovered hundreds of instances where I have crimp lugs with a 3/8" bolt hole connected to another lug (motor leads) or breaker stabs, which have 5/16" bolt holes.  All are silver plated copper, I am confident of tight connections, and thermal surveys have been very good. The hardware used is 5/16" bolts with flat and lock washers.

  Not ideal, and I wouldn't have done it. But should I really go thru the expense of fixing this? Equipment can't be easily shut down and some are on the other side of the world (shipboard). I have no regulations (such as LLOYDS) that tell me I can't do this. TnB has not responded to my question yet.

Opinions?

The answer is in the thermal analysis.  If that indeed checks out don't bother re-fitting.  Just correct them during replacement.
