I'm looking for a pneumatic valve (to feed air to a on/off valve actuator) that is manually operated with maintained lever or pushbutton that will return to the failed state on loss of air pressure.

any help?

Any valve with just a pnematic operator acn be fitted with such a solinoid.  Look at Versa-valve.

Use a three port two way solenoid that will route air to the actuator when in the energized position and vent air from the actuator in the de-energized position.

Almost any valve is available with a single-acting actuator: Air strokes the actuator one way, remove the air and spring(s) return the actuator/valve to its original position.  

You didn't say anything about your service.  You have to select the valve for the service, then select the actuator required to move the valve in that service.  

ASCO ( a big name in solenoid valves) calls the function you want "3-way, normally closed".  Energize the solenoid valve and the air flows through from the supply port to the actuator.  Deeenergize the solenoid valve and the supply port closes, and a vent port opens, dumping the air in the actuator to atmosphere.  


Here is a link to Automax actuators.  These would be a good choice for automating a ball valve or butterfly (rotary valve) for your isolation application.
