Does anyone know how to underline dimension text in SW.  This is for dimensions shown out of scale (see below).

ASME Y14.5M-1994. "Dimensioning and Tolerancing", paragraph 1.7.9 states that, for a dimension not to scale, the defining dimension is to be underlined with a straight thick line.

I can't find a way to do this and would appreciate any suggestions.

Uncheck "Use Document", click "Font", and choose "Underline"

... but forgo the thick line part. ;)
Unless you want bold font.
