Is it possible to couple with flange adapter  reducer with pipe? This is the case where leaking has occured at this joint.

I don't know what kind or size of pipe you're dealing with, but it should be possible (in theory) to get a reducer with flange couplings on both ends.

If you can't find one, you can always fabricate one.
