I have come across failure of a 3cx240sqmm, 1000V rated, armoured, XLPE insulated, PVC outersheathed power cables recently. The failure shows that the PVC outersheath over the armour is peeled off.
This has happened after about one year in service. Can this be some thing to do with the outersheath material composition!

Possibly, but more likely it was damaged during installation. Can you provide more details.
