   How can I machine a titanium conical screw which has a taper angle 22 degree and a pitch 0.8mm (Number of revolutions=4)?

A lathe with a taper attatchment should do the trick.

you didn't say if the 22 deg angle was included or single side. if single side, I doubt a taper attachment will accomodate you very well.  this is easiest to do on cnc.
