I'm currently evaluating NX5, so my first questions are:
1) How do I set the navigators and pallet menus to the R hand side of the screen versus the L side as installed?
2) Why won't it let me dock icon menus on the sides?

Ok...now back to reality.
Were currently on NX3, with the worst possible CAD environment you could think of (no associativity anywhere...) and about 15 total users at the moment. For me (read both the CAD side and individually) it's a no brainer: Migrate to NX5 now. But it's not just me... I need to consider the rest of the world....
Should I: 1) Stay NX3 2) Go NX4, then NX5 later, or 3) Go NX5 now? ...and why? ...and considering the above tirade, what would you suggest?

Here where I work, we are still on NX3 because we do a lot for GM. But what we send is mostly parasolids (why not use NX5 and just ship out older versions of parasolids, well I'm trying to get this).
I wouldn't go NX4 since NX5 is out. No sense in doing 2 separate transitions.
And it only makes sense to get into a fully parametric environment. I am a huge advocate on this. Finally I have shown the couple guys I work with the advantages of a fully parametric environment. Sketches, datums, etc. Makes the modifications so much quicker. We do suspension bushings and brackets. It's a couple cylinders with rubber in between, wave linking geometry to keep things concentric and maintaining certain distances, now that's what I call a good time.

I'm VERY impressed with that particular feature, and you KNOW I'm not afraid to be a critic when "needed". I also feel that development has made a change for the better in terms of at least considering what users are saying on the boards. I don't mean to imply that they never did before, but it just seems like there is a better understanding of what the users would like to see and what development is willing to at least try.

We currently aren't using NX for anything other than modeling (no internal Teamcenter or anything and no CAM or mold making) and so far, NX5 has been a remarkable improvement over previous NX versions.
There are a few bugs, but the ones I've ran into are far from show stoppers. I had more bugs from NX2 to NX4 than any other version going back to UG v11. Granted, my modeling techniques and knowledge changed from the early days, but none the less, most of our issues came after NX1.
I would encourage you to go to the BBS Notes and browse through the CAD forum to see if there are ANY issues that would affect you if you were to migrate to NX5 now. Some of the issues I've seen aren't part of our workflow but may affect yours or what you might envision directly.
Based on the modeling and drafting that we utilize, I'd say move to NX5. It's only going to get better from there.
