In my daily travels around Power Generating stations and with pictures like this, it really makes you stop and think how we (I) sometimes take for granted this type of equipment operation. A failure occurs and instant death or permanent injury.

I always remind myself that we don't pump chocolate sauce in our pipelines, mother nature always tries to get to the lowest energy, most chaotic state as proven by the picture.

It's amazing that the energy release was enough to essentially unwrap the entire thick steel tank.  I would expect more of a blown out seam or untucked end seam.

I can tell you I won't be standing next to these reservoirs anymore. (as I have in the past)

Greetings all,
I saw a failure similar to this fresh out of college when I was doing insurance inspection/claims work.  Consensus opinion was excessive porosity in the centerline of the longitudinal seam with failure starting at head/shell attachment weld.  Tank basically just "unzipped".  Good picture which I will add to my archive.
