i hope someone can help

we want to pour a new concrete slab 5" thick rienfoced with 2" fibermesh on top of an existing slab.we are thinking of placing poly on top of the existing slab so the two slabs are isolated.the existing floor is 6" thick,over 50 years old and has a lot of surface wear.New slab is 15,000 sq. ft. and would be sawcut into 15' squares.New floor will have forklift traffic on it.There are 6 areas 6' x 6' that are broken areas and we want to install re bar mats in these areas without removing the dameged concrete.

your help would be greatly appreciated.
