When placing rebar for vertical members, is a contact lap splice always required?  Is there a tolerance on this?  I have already read ACI-117 section 2.2, but have not fully understood all of the various points therein and will cogitate upon them with guidance.

When placing pre-tied column cages on the dowels of the previous lift sometimes the dowels may have been sqoze in a little too tightly and now there is a couple of inches between the dowels and the verts of the new cage.  Or a section of pre-tied wall resteel the dowels may be off a little from the verts of the section be flown in.

All vertical bars require a full contact lap splice or is there a tolerance for distance between the these bars?  And if yes on the tolerance what part of ACI-117 2.2 tells me this?

ACI 318-05 12.14.2.3 gives maximum distance between non contact lap splice bars.
