- Is it possible to eliminate the revision format of A-00, A-01, A-02 and replace it for something like T1, T2, T3, etc?

If you're talking about PDMWorks, yes, you can do that.  However, the revision scheme changes will only take effect on newly created documents, not existing ones.

PDMAdmin is correct. If you have to change the older revs, you need to check them out, delete from the vault, change the rev settings (admin), then check in as new revs, in steps from old to new.
The problem is if you have any type of configuration management system, you will not have the record of the old revs.

sounds like a lot of work but seems to me its the only way out.  

Currently we keep revisions by date, by A-00 levels, also by v1.0 and by SKU.

no one ever decided on documentation control and now its a huge spaghetti bowl of confusion...

If not done correctly of front, then decide later to make it right ... then yes it can be a lot of work and confusion.
