I am looking for a simple engineering change form. I want to start a very simple ECO system and form. I would like to keep the return on investment short and sweet, or even on a separate document.
In the past I have been accustomed to a request for change that is submitted before the ECO is granted. At this time the request for change could be skipped in order to keep it simple. You might have some ideas for a simple method and form.

For example if you do work for the UK Ministry Of Defence they have a preferred layout (they usually use the terms Amendment & Modification rather than ECO). Talking to a colleague though I'm not sure this is true in the US. The MOD based amendment form we used at my last place had the following fields(slightly abbreviated):
Title of Equipment
Sheet No.
No. Sheets
Amendment No.(tor you ECO)
Compiled by
Checked by
Designers Approval
Date of Embodiment
Initiated by (for you purpose this would probably be the ECR ref) Reference & Date
Approval for issue, Reference & Date
Drawings affected, Drawing sheet number & new issue (Rev) No. (this was a summary list)
Reason & General Description
Drawing NI, Description followed by details of alterations. (This was a detailed list with all changes explained in enough detail to allow the drawing to be reverted to previous rev)
Note this form only concerned itself with the drawing pack (including item lists and drawing lists) not any effect on manufacturing BOM etc.
From what I’ve seen commercial companies tend to be less demanding than aerospace/defense on how detailed the form need be although they may have their own additional requirements.
