I just discovered today that there exists a UV-1 form for ASME Code relief valves. I have been working with relief valves dor about 20 years, and this is the first I have heard of the form's existence. I have never received one when I purchased a Code relief valve. How long have these forms been around? I did not find any references in Sec.VIII Div.1 2001 edition, but it appeared in the 2004 edition.... I'm puzzled?
Also, ASME Sec.VIII Div.1 calls for a UV-1 form, while Sec.VIII Div.2 calls for an A-4 form; both forms appear identical except for the Div. number. Is there a difference between Div.1 and Div.2 relief valves?

These forms are a result from revisions to paragraph UG-117 of the 2003 Addenda for which a Certified Individual is required. The requirements for a Certified Individual (CI) were developed to comply with the provisions of an ASME Codes and Standards policy document, CSP-53. This document requires a CI to have an acceptable understanding of ASME Code requirements such that he or she will be held responsible for applying the Code Symbol and certifying compliance of Code pressure objects on ASME Code documents in circumstances where an Authorized Inspector (AI) is not involved in final certification. ASME's goal is to protect the integrity of the "Mark" , or the ASME Code Symbol, where third party oversight is not mandated, such as for "UM" vessels or "UV" and "UD" pressure relief devices.
