In the USA, can a PT Slab have a Banded/Banded type layout?  Does ACI 18.12.4 apply to PT Slab design which basically says you can't have Banded tendons in both directions.

Banded-Banded is not allowed - as pointed out by your code reference.  Banded-Uniform is typical.

You have to use a load balance layout according to ACI so banded in both directions is not allowed.

Banded in both directions can be made to work as long as you reinforce the slab between the bands as RC middle strips for the moments that occur there.

How do I design a PT slab from first principles? What is the difference between a PT slab and a prestessed concrete beam (which I can design)?

I think the detail required to answer this is a bit beyond questions to a list service like this. Find a good book on the topic (and that does not include the ACI code or any PTI documents).

If you understand how a PT beam design works and an RC slab design, you should be able to put the 2 together.

The Prestressed Concrete Institute had/has a publication on the design of concrete Post Tensioned slabs.

I used Lin's old text on prestressed concrete and my current text is "Prestressed Slabs" (I think), I'll check when I get to the office.  I think I can still locate my dogeared PCI manual.
