Not that I'm opposed to short-cuts and here in the US, after almost 10 years have past since OSHA required existing facilities to have their initial PHAs complete, you would think that facilities would have good established programs that they could look for ways to reduce the burdens without sacrificing safety. However, it seems I'm still seeing and hearing of traditional PHAs (even going into their third cycle) being scraped because they were deemed inadequate. In one respect, I suppose that's the result of a better understanding and training by the ones following in the foot-steps but it may also be a failure of recognizing the importance of managing changes that occur during the cycle.
Of course, traditional PHA or new MHA, there's always potential for not properly applying the method.
Okay, enough on the soap box, Primatech is the software of choice at the facility where I'm located so you may want to look around their website for more info about traditional PHAs. As 25362 implies, a web search will lead you to a variety of places where you will find info, you'll just have to spend some time searching to find what your looking for and checking the software vendor sites is a good start.
