Simplest way is to open one of the SW default templates & modify it to suit your companys standards.

We came from Acad world and followed Xref style of doing things. We have changed our company name and logo 3 times since starting SolidWorks 98.

Leaving the company logo and name off? I'm not so sure about that. Sure, logos and names can change, but normally updating of already existing drawings is not a requirement unless the drawing is up for some other change.
Statements covering tolerances, propriety, type of project, company name and address, sheet # of #, sheet size and file name are common. (link to SW's standard properties for these last three, and they will automatically fill in the correct info)
Empty or linked fields that are common are material, finish (and/or finish # if working with a lot of machined metal parts), Revision, title, drawn by. Create custom properties on the template and link these annotation notes to these properties (also on the template)
Don't forget the revision block in the upper right corner. USe SW's Revision table to create this on the template. Modify to suit your company's needs.
Don't bother with any approval fields if your company has a paperless system. They'll never be used.
These are just some thoughts. I'm sure a lot of people here have plenty of history in this area. :)
