I am looking for a good design example of a bolted base plate using AISC.  Also, if anyone has recommendations on a spreadsheet for this design, it would be appreciated.

Assuming you are asking about a base plate with applied moment, a trustly reference is Omer Blodgett's classic, "Design of Welded Structures".  Also, AISC's Design Guide #1 is about column base plates.

Steel Structures: Design and Behanvior by Salmon and Johnson also has some good information for designing base plates.
