Hello,
Is there a list of pressure rating vs. pipe thickness for all types of piping anywhere (or a calculation)?  Any standards on this that I could reference?

No. Piping is supplied by OD and schedule. If you want to determine the necessary wall thickness for your application, this should be done using one of the ASME Pressure Piping Codes or for water service applications use the AWWA Standards. These Codes have formula that will provide for minimum wall thickness based on service temperature and pressure. Using Codes and Standards will assure adequate design margin (taking into account pressure and temperature) and aid in the selection of proper material for service.

It depends on your application. Go to the ASME.org web site and review their list of Pressure Piping Standards -
ASME B31.1, B31.3....etc

You will find listings and ratings of most of the piping materials (pipe, flanges, etc).  As metengr says, you should be cognizant and knowledgeable in the applicable ASME, ANSI, API, etc. codes before applying these ratings to your piping systems.

Is there a minimum pressure in ASME B31.1 below which the piping is not considered "pressurised"?

Yes, carburize. Per B31.1 - scope - 100.1.1

boilre external piping where steam or vapor generated above 15 psig and
hot water above 160 psig pressure and 250 deg F temperature
