I've seen these but I don't know where.  My specific needs are 0-50 gpm, 50-150 degF and 0-100 psi.  2" pipe connection would be perfect.

If you have enough money, Emerson's MV3095 multivariable flow transmitter would do the job. Not a cheap instrument, but reliable and compact. There are a multitude of options on these devices - discuss the specifics of your application with their engineers.

The MV3095 is the right instrument.  You will have to read the information by a HART reader or add to it a HART to 4-20 ma translator Emerson TRI-Loop which connected to the transmitter in Hart protocol and has 3 outputs of 4-20ma.
SMAR and Yokagawa has similar instruments as well.

Thanks for the feedback but these are in a different league than the one I was looking for.  Some time ago I ran across a paddle wheel type flowmeter.  It was mounted in a square metal block with a window on the paddlewheel.  The block had 1" npt pipe connections.  Intergrally mounted within the block ther was a pressure and temperature sensor. Electrical connection was with a 6 wire cable which I think contained 3 4-20ma signals, 24vdc and ground.
The 1" device was under $1500.
