I am working with a PCB assembly that is using unusual fasteners. These are #0 Phillips drive M3x0.5 thread - 5mm thread length but with head flat on both sides not PAN, CHEESE, TRUSS or anything I find, also very low profile. Looks like a 90° flat head but with the 90° gone. Also has blue patch for self-locking...

More information:
Head OD = 5.25mm
Head Ht = 0.8mm
OAL = 5.8mm
Phillips size 2.4mm cross, 0.8mm wide, ~1mm deep

I've googled to no avail. Any ideas where to buy 100-3000?

Consider having it made as a custom part.  You'll probably need to buy 10,000 or so, which sounds like a lot, but ...

...  custom screws get really cheap when you buy enough to keep the machinery up to speed for a little while.

... ten thousand of those screws won't fill a coffee can.

Those dimensions look a lot like many of the special design screws that are used in various electronics applications.  You will have a very difficult time finding those types of parts available off the shelf ar they are virtually all made to a customer print.  The head dimensions and other tolerances are designed specifically for the application at hand.  
I second the idea of drawing up a print and having them made up.  My best guess is that the lot charge for 3,000 pieces would be about $1,000. Maybe less if they are carbon steel without heat treat
