Please help me to find some good "failure analysis reports" examples in order describe better for a defective electronic circuit board.

Sources can be web sites, books or research works.

This is a customer in automotive sector.

Guidelines and real examples are good helpful!

Perhaps your customer can provide you with a "format" in which they would like to see the report done.  A couple of sites below have some examples.

At a minimum, I have seen the basic format of:

Statement of the problem from the point of view of the customer.

Conditions in which the failure occurred

Analysis of the failure mechanism(s) (root cause)

Corrective action plan for future prevention

Appendix with supporting data.

I have done several failure analysis reports in the past and have used a variety of formats.  The most effective seems to cater to the audience.  If you are providing the analysis for techs, then be very technical and throrough.  If it is for execs, I try to use more graphic representations (charts and graphs) that are easier to follow then written words.  Most important detail is to be extremely accurate no matter the format, your data will be scrutinized by those that are requiring the analysis.
