A 11th stage pump is running with 10th impellers. In case of adding a last stage impeller, am I have to change the balancing drum ?

I would think - Yes - but to be sure why don't you check with the manufacturer.

Has it always run with 10 impellers?  So that the balancing drum might have been initially designed for only 10 impellers, or was it modified along the way by removing an impeller?

Knowing this might have some bearing on the answer.

10 impellers were installed at the beginning, but added last impeller due to head requirement. Motor was originally selected on the basis of 10 impellers. I wonder the balancing drum was designed for 10 or 11 ?

I'm afraid not having a crystal ball to answer if the unit was desigend for 10 or 11 impellers, you will have to ask the manufacturer or the supplier.
