Is the axial force of a worm the same as its output force on the worm gear?  If not, how can I calculate the output force on the worm gear by the worm?

Yes, this is the force applied to the teeth of the wormgear by the worm. This force has an equal and opposite force axially on the worm which is transfered to the thrust bearings at the end of the worm shaft.
