"Fatigue and Fracture in Steel Bridges" by John Fisher. Dr. Fisher did much of the research that went into the AASHTO code. AISC throught the National Steel Bridge Alliance also publishes an excellent book on the subject.

The book by Fisher gets more into the theory. The NSBA publication deals more with the application of the code; it contains a number of sample problems.

Both books are well worth the money.
