We are currently in the process of analyzing a steel base plate for a telecommunications monopole, and have stumbled across a reference to Horn Formula A. Does anyone have any information they could provide on this? If so, it would be more than appreciated.

There is a standard, ANSI/TIA/EIA-222, “Structural Standards for Steel Antenna Towers and Antenna Support Structures.” It apparently includes base plates. I've never seen it, maybe that's what you're reading in. Or maybe it contains the elusive formula!
It is maintained by the TR-14.7 Committee of the TIA (TIA- Telecommunications Industry Association, tiaonline.org).
Mr. Dan Horn, deceased, was apparently a member of that committee. He was President of C-concepts, Inc. www.c-conceptsinc.com. One of the items listed on their website is the 192-page user manual for a tower design program, the user manual being written by Dan Horn.
I still don't see a "Horn Formula" in all this, but maybe that'll get you closer. Good luck.
