I am looking for an equation(s)/charts/references to determine at what distance a flashover would occur between phase/phase and/or phase/ground.  The equations would need to take into account atmospheric conditions, sag, etc.

We are trying to recreate an outage where one 15kV circuit sagged into another 15kV circuit.  An initial conclusion is that the distance will be pretty small (<1").  

I've only been able to find information for higher voltages (+100kV).

14th Edition, McGraw-Hill, 2000,
1a. Corona and Breakdown in Nonuniform Fields between Conductors on page 4-141
1b. Section 27 "Lightning and Overvoltage Protection" is somewhat related regarding surges that can cause the flashover.
2. There are also other books dealing with this topic, and some postings in this Forum
