bolt & nut failure

(OP)
Gday folks,

am wondering if anyone has some neat ideas to retain bolts/nuts in the event of a failure, to prevent them hitting someone if they have failed under tension. short of welding snug blocks around and over them, i am not aware of any other ideas

RE: bolt & nut failure

Another method is to have a small bolt throught the center of the larger one.  This help retain the bolt, but does not necessarily allow one to know if it has failed.
