Problems with smart fasteners

(OP)
I am having some trouble with Smart Fasteners (SF). I have a plate with 2 tapped holes in it. The plate has 2 configurations (right hand side and left hand side). The holes are the same size and positions in both configs. The way I created 2 holes was this: I created one hole using Hole Wizard and then feature patterned it to get the second hole.

Now I tried to insert fasteners in these 2 holes using SF. I first did this for the first config. The smart fasteners put fasteners successfully into both holes.

However, when I tried to do the same thing for the second config, it put fastener in only one hole (the original hole)!!! I tried to add the second hole manually but it won't select the second hole!!!

I am confounded as to why this is happening. If it works for one config, why doesn't it for the second config (which is the same as the first config as far as the size and location of these 2 holes go).

Your suggestions will be greatly appreciated.
Thanks
