why do not extrussion between two faces sometimes work properly?

Need a little more info that just that in order to get an idea of what you're trying to accomplish.  It could be operator error or a bug in the programming.  Without knowing what steps you are taking, we can't eliminate possible causes.

I don't exactly know why this is but it's one of my main gripes with UG. Whenever I see the option to Add or Create during an extrusion, I usually use create because I've gotten used to not expecting UG to complete the task.

If you create your extrusion so that it extends through the faces you want it to go upto, you can do a surface trim later on to get your desired geometry.
