Biaxial Bending of Concrete Column / Beam

I have a concrete frame with biaxial bending in the beams and columns.  I'm wondering if anybody knows of any GOOD literature or examples on this topic.  I've been trying to go through an example by Edward Nawy but I've been getting lost in the process.  Basically, my moments are high relative to the compression in most cases but I can't find anything on biaxial bending of a beam with negligible axial load.  

I appreciate any help.

"Reinforced Concrete" by Warner, Rangan and Hall (ISBN 0 582 71226 2) addresses biaxial bending in both beams and columns and has worked examples.  They reference the following paper in the chapter that includes a treatment of biaxial bending in beams:

   Vol. 98, No. ST5, 1972.

The Australian Standard, AS3600 "Concrete structures" addresses combined actions in reinforced concrete columns.  Some papers referenced in AS3600 are:
