There are a number of small assemblies in our PDMWorks vault (2004, sp1.0) that even though previous revisions list the references correctly, the latest rev lists no references.  When you "Open Document" from the vault, none of the parts referenced by the assembly appear in the references box.  

The only unique thing I have found with these assemblies is that the assembly was checked in at a different time than the parts.  Also, subsequent revisions were made to the parts, and checked in without updating the assembly.

I have been unable to reproduce the problem in a test environment, short of deleting all the parts in the assembly and checking it in (this is not what happened).

Can anyone think of a scenario that might "detach" an assembly from it's parts in the vault?

That sounds like it could be vault corruption.  Are you aware of any vault files that have been manipulated without the PDMWorks client?  (That's a really bad idea, but I've seen it happen, and the results weren't pretty.)  Your best bet is to load the assembly, make sure the necessary components are mated correctly, then check it back in.  Not the fastest or most elegant solution, I know, but based on the info you provided, that should fix your problem.
